How do production threat kids in horror movies?

This is a question I got myself into after watching the new It movie.

If a film like that (and any other horror film) is rather R for some reason, how do they get to have kid actors in it? Especially if they appear in some gore scene, I wonder how mature those kids must be, or...don't they know what's going on with their role or something?
